Volunteer Application

Please note: No further volunteer applications are being accepted at this time. We are grateful for the many generous individuals who have responded to our call and will serve at Camp Forget-Me-Not!

To become a volunteer at Camp Forget-Me-Not you must complete two forms:

  • Volunteer Application Form
  • Criminal History Background Form

To submit your Criminal History Background Check Form, click here to print a copy of the form to submit by mail.

Please read the following requirements carefully before submitting the Volunteer Application Form.

  • Camp Forget-Me-Not will conduct a Criminal History Check on all volunteers. By completing this application you will give permission, and provide specific information (such as your driver’s license number and social security number), which will be used ONLY to secure information regarding a “conviction only” criminal history. This information is kept strictly confidential and will be used only to check criminal history records. Any information obtained by Camp Forget-Me-Not will also be kept strictly confidential.
  • Volunteers must be at least 18 years old and must attend two training sessions.

Wednesday, May 26 at 5:30-8:00 pm
and
Wednesday, June 2 at 5:30-8:30 pm

The training sessions will be held at Ele’s Place and a light supper will be served. If you have previously served as a volunteer at Camp Forget-Me-Not, you are required to only attend the second training session, although you are also welcome to attend the first session.

  • Volunteers are required to have a TB vaccination, which will be provided at no charge during the second training session, and will be read on the evening of June 4 (the first camp session at Ele’s Place). Volunteers who have had a TB test within the last year are not required to have another test, if they provide written proof of a current negative TB test.
  • Volunteers are asked to provide two references, and these references may not be relatives. References may be friends, neighbors, co-workers, or other colleagues.
